Youth Transitioning to Community Rehabilitation and Treatment

The following protocols/guidelines are intended to make the transition of youth who are receiving services from a Designated Agency Children’s Program or are in custody of Department for Children and Families (DCF) and are likely to be meet criteria for Community Rehabilitation and Treatment(CRT) services as seamless as possible for the youth and family.  Please note these guidelines begin upon the youth’s 17th birthday if it is deemed likely the youth would meet CRT criteria by their 18th birthday.
